Koakuma is an adorable little devil girl that somehow got found working with Patchouli. According to canon, she's a mischievous fairy-like prankster who must pull lots of pranks in her free time, given that they don't involve her master's work.
For some reason, she has bat wings as ears. But some people prefer her with them wings on the back instead. Inaba of the Moon & Inaba of the Earth chapter 23 even shows her with both! But other than that, we don't really know who she really is. Whether ZUN will give her another appearance like what he did with Daiyousei, a fellow midboss from the same game she appears in, only time will tell.
Etymology
The name Koakuma was never (intended to be) official; she was originally just an unnamed mid-boss, and the name was taken from some label in the game's code. It literally translates to "little devil".
