A Poison mushroom, sometimes called a Purple mushroom, will either shrink you to half you height or kill you if you are already shrunken. It is suggested to only eat them when you want to be shrunken and know there is a healthy supply of red mushrooms nearby. They are quite the opposite of red mushrooms.

Poison mushrooms are often found in question mark boxes, like Red ones, making them even more tricky to avoid. Poison Mushrooms have three key differences from regular Red and Green mushrooms. The first is that they usually have a death to you face. The second is that they are usually purple in cap color. Finally, Poison Mushrooms have evil skulls adorning their caps instead of white spots, clearly stating their supposedly toxic nature.

Poison Mushrooms do not appear in either Mario RPGs or Mario Kart games. However, Poison Mushrooms appear in Mario Party 2 and Mario Party 3. Here they are an item that will make the Dice block only roll up to a 3. They also appear in the minigame "Rake 'em in", where they appear with other normal Mushrooms and subtract points from the character should they rake them into his or her basket. Many Poison Mushroom-esque items also continue to appear in Mario Parties.
